RN Clinical Liaison
Hospice Plus
Hospice Plus is seeking an RN Clinical Liaison to connect patients, families, and referral sources with compassionate hospice services. In this field-based role, you will assess patients for hospice eligibility, explain services and care options, and coordinate smooth admissions. You will collaborate closely with physicians, discharge planners, and our interdisciplinary hospice team to develop individualized plans of care. The RN Clinical Liaison educates the community on hospice philosophy, maintains accurate clinical documentation, and advocates for patients to ensure a dignified, comfortable end-of-life experience.
