Account Executive - Tucson Are you a driven, relationship-focused professional with deep knowledge of Arizona's home and community-based services? Total Care Connections is seeking a highly motivated Account Executive to join our Tucson team. If you have a proven track record of generating client referrals - especially from Banner, the VA and Private Pay clients - and you thrive in a role where results matter, we want to meet you! Our ideal candidate is passionate about serving seniors and disabled adults, excels at connecting with referral partners, and understands the rules, regulations, and requirements of Arizona Medicaid HCBS programs. Job Summary The Account Executive plays a vital role in growing our client base by cultivating strong referral relationships, conducting timely client assessments, and building individualized care plans. This is a position that will require you to be in office at times and also out in the community.
Success in this role requires self-motivation, consistent follow-through, and the ability to meet clearly defined performance targets. Duties and Responsibilities Referral Partner Engagement
• Develop and maintain strong relationships with Banner, the VA, as well as Private Pay community referral sources.
• Manage several key accounts.
• Conduct at least 50 outbound calls per day to referral sources and/or prospective clients.
• Send follow-up emails for every referral and a thank-you card to every referral partner.
• Track and meet weekly referral growth goals set by leadership. Client Intake and Care Planning
• Conduct in-home or on-site new client assessments and meet with families to identify needs.
• Create comprehensive, individualized care plans that align with assessment findings.
• Ensure care plans are clear, goal-oriented, and initiated promptly after intake.
• Communicate care plans to caregivers and operations team members for smooth service initiation. Compliance and Documentation
• Ensure all intakes and care plans meet Arizona Medicaid and company standards.
• Maintain accurate, timely documentation in the agency's systems.
• Understand and apply eligibility requirements for Banner and the VA. Collaboration and Communication
• Partner with Care Managers and Scheduling teams to ensure new clients are staffed quickly.
• Represent Total Care Connections professionally when meeting with clients, families, and referral partners.

• Preferred: Experience working with Banner and the VA referral processes.
• Experience bringing in Private Pay clients.
• Experience managing multiple key accounts at one time.
• Preferred: In-depth knowledge of Arizona Medicaid HCBS regulations.
• At least one year of experience in the home and community-based services industry in Arizona.
• Proven ability to generate and grow client referrals from professional partners.
• Excellent communication and relationship-building skills.
• Strong organizational skills with attention to detail.
• Ability to work independently and meet high daily/weekly activity targets.
• Proficiency with computers, CRM systems, and email communication.
• Reliable vehicle and clean driving record.
