FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier home health care for adults in the Hercules area. This Independent Contractor role focuses on functional assessment, ADLs, and safety in the home, with a plan of care tailored to each patient’s needs. The OT will design and implement treatment plans, perform therapeutic interventions, and regularly evaluate progress, while engaging patients, families, and care teams to maximize independence and safety at home. #J-18808-Ljbffr FeldCare Connects
