Staten Island, NY Personal Touch Home Care is expanding our Special Needs Program and seeking compassionate Occupational Therapist who are passionate about delivering high‑quality care in the home setting. This is a fee‑for‑service position, paid per visit and this is a 1099, contracted position. To be considered, you must have a valid and active license in the state of NY. Job Overview As an Occupational Therapist with Personal Touch Home Care, you will assess patients' needs, develop and implement treatment plans, and help clients improve their daily living and work skills. You will provide rehabilitative services to patients who are unable to travel to a clinic or hospital, focusing on improving patients' quality of life by promoting mobility, function, and independence within the comfort of their own homes. Responsibilities Assess referred patients that require rehabilitation services. Provide direct patient care to assess their medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician. Set realistic and achievable goals for patients. Document and record patients’ condition. Educate patients and families on an appropriate therapy method. Prepare evaluations, weekly progress notes, recertification, and discharge documentation. Collaborate with all disciplines to provide the best patient‑centered treatment and care. Perform all other duties assigned by the Director of Rehabilitation. Comply with confidentiality, HIPAA and Federal Residents Rights. Qualifications Graduate from an accredited Occupational Therapy college or university. Hold a valid professional license and registration for the state of practice. Experience with long‑term care and/or geriatric care is preferred. Demonstrates good verbal and written communication and organization skills. Prefer two (2) years of appropriate experience as an occupational therapist; home health experience is preferred.
