Seeking a licensed Nurse Practitioner (NP) or Physician Assistant (PA-C) with primary care, internal medicine, urgent care, or ambulatory care experience to provide comprehensive patient assessments, diagnosis, treatment, and chronic disease management in an outpatient setting. Responsibilities Assesses patient health by interviewing patients; performing physical examinations, updating, and studying medical histories.

Practices using current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment.

Determines abnormal conditions by administering or ordering diagnostic tests, such as x-rays, electrocardiograms, and laboratory studies; interpreting test results.

Documents patient care services by charting in EMR (Allscripts)

Performs comprehensive or episodic health assessment of patients/families with acute and/or chronic health problems with an emphasis on wellness. Preserves and promotes good health through education and counseling patients in positive health behaviors, self-care, and coping skills.

Performs therapeutic procedures by administering injections and immunizations.

Renders patient care with high degree of service excellence, which will be measured by pre-determined targets.

Instructs and counsels patients by describing therapeutic regimens; counseling and managing chronic conditions; providing counseling on emotional problems of daily living; promoting wellness and health maintenance.

Maintains safe and clean working environment by complying with procedures, rules, and regulations.

Contributes to team effort by accomplishing related results as needed.

Training and Experience Requirements: A Master's degree and certification by Physician Assistant or Nurse Practitioner certifying body is required. Must have and maintain a DEA license with no restrictions.

Clinical experience in an primary care, adult medicine, urgent care, or ambulatory care setting is preferred.
