Per Diem Registered Nurse (RN) - Oxford, Chenango

We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our per diem home care team in Oxford, Chenango. As a Per Diem Home Care RN, you will play a crucial role in delivering high-quality patient care and proper supervision of home health aides in the field. With a focus on flexibility, independence, and personalized patient interaction, our home care team is dedicated to making a meaningful difference in the lives of our patients and their families.

Responsibilities:

• Conduct initial patient assessments and develop individualized care plans
• Complete UAS assessments
• Perform routine supervisory visits for home health aides
• Orient and educate caregivers on new patient cases and care expectations
• Complete 6-month recertification assessments
• Ensure accurate and timely documentation in compliance with NYS and agency standards
• Communicate effectively with the clinical and administrative team to support quality patient care

Requirements:

• Active, unencumbered New York State RN license
• Home care or community health experience preferred
• Experience with UAS assessments preferred
• Strong clinical assessment and documentation skills
• Reliable transportation and ability to travel within an assigned county
• Comfortable working independently in the field
