Restorative Nurse ( LPN )

Lineville Health & Rehabilitation, LLC is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team as a Restorative Nurse . In this role, you will help residents maintain and improve their highest level of physical, mental, and psychosocial well-being through individualized restorative nursing programs.

Responsibilities:

• Coordinate and oversee restorative nursing programs for assigned residents.
• Collaborate with nursing staff, therapy services, physicians, residents, and families to develop and implement restorative care plans.
• Participate in the MDS/RAI process and interdisciplinary care planning.
• Monitor resident progress and communicate changes or concerns to the appropriate healthcare providers.
• Initiate therapy referrals and serve as a liaison between the nursing and therapy departments.
• Ensure restorative services are delivered in accordance with physician orders, resident goals, and facility policies.
• Educate residents and families regarding restorative care programs and goals.
• Maintain accurate documentation and compliance with all federal and state regulations.

Qualifications:

• Current LPN license in good standing.
• Knowledge of restorative nursing principles and long-term care practices preferred.
• Strong communication, leadership, and organizational skills.
• Ability to work collaboratively with an interdisciplinary team.
• Compassionate, patient-centered approach to resident care.
