Registered Nurse DaVita Skilled Nursing Dialysis (DSD) Premiere Washington Rehabilitation and Nursing Center - 36 Old Hickory Ridge Rd, Washington, PA 15301 Schedule: 3 days a week, 12+ hour shifts; no on call. Training provided. Alternating Saturdays required. Some travel to other SNF clinics within the region may be required. Job Overview DaVita Kidney Care is bringing our industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through the DaVita Skilled Nursing Dialysis Services (DSD) team. This role provides hands-on and classroom training, and you will quickly build infrastructure and growth for this new service line. Ideal teammates have dialysis experience, thrive in a start-up environment, and excel at autonomous work while collaborating across functions. Responsibilities In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. DSD RNs troubleshoot and make sound decisions for patients, and occasionally partner with SNF staff to coordinate care. Administer dialysis care independently, leading a team of Dialysis Technicians. Educate patients and families in the SNF setting. Manage complex care demands, multitask, and monitor clinical outcomes to exceed program goals. Apply critical thinking, assess, diagnose, and implement comprehensive care plans. Support other local SNF sites as needed. Qualifications Current RN license in the state of practice; minimum 12 months RN experience. Associate’s Degree in Nursing (A.D.N) from an accredited school; Bachelor of Science (B.S.N) or accredited diploma accepted. Basic computer skills and proficiency in MS Word and Outlook. Previous chronic hemodialysis or acute dialysis experience preferred but not required. Paid hands-on training program provided. Desirable certifications: Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN). Background in training, teaching, or healthcare education is a plus.
