About the Role:

As a Certified Home Health Aide (HHA) with SYNERGY HomeCare, you will have the unique opportunity to make a meaningful difference in the lives of our patients. You will provide non-medical care and assistance to patients in their homes, helping them maintain their independence and dignity. Our HHA team members are trained to provide exceptional care, compassion, and companionship to individuals in need.

Responsibilities:

• Provide non-medical care and assistance to patients, including activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Assist patients with personal care, such as bathing, dressing, and grooming
• Assist with light housekeeping, laundry, and meal preparation
• Provide companionship and social support to patients
• Monitor and report any changes in patient condition or needs to the supervising healthcare professional
• Maintains accurate records and documentation of patient care and services
• Collaborates with other healthcare professionals to ensure coordinated care and services

Requirements:

• Must be a Certified Home Health Aide (HHA) with a valid certification
• High school diploma or equivalent required
• Ability to lift, transfer, and move patients, as well as bend, stoop, and stand for extended periods
• Ability to communicate effectively with patients, families, and healthcare professionals
• Ability to work independently and as part of a team
