Occupational Therapist, SNF — Kent, WA. This travel, temporary assignment begins July 13, 2026 and runs for 12 weeks. The role supports patients in a home care environment, providing coordinated OT services across ages and varying diagnoses.

Responsibilities
- Perform clinical assessment to identify patient needs and determine appropriateness for occupational therapy.
- Document, plan, implement, and revise a Plan of Care (POC); maintain progress notes and other clinical records.
- Deliver direct therapeutic interventions addressing ROM, strength, coordination, adaptive equipment use, and environmental modifications to support ADL performance; fabricate and maintain adaptive equipment and hand splints as needed.
- Evaluate patient occupational performance across self-care, work, leisure, and rest; assess sensory-motor, psycho-social, and cognitive-perceptual components; consider environmental and cultural factors.
- Apply infection control and universal precautions; coordinate care with the interdisciplinary team; consult with physicians regarding patient status.
- Communicate relevant patient information to physicians and caregivers while protecting confidentiality; maintain accurate records and assist with third-party payer education as needed.
- Troubleshoot and perform basic maintenance and setup for OT equipment; participate in related duties as assigned.

Requirements
- Current state license as an Occupational Therapist (active)
- CPR certification (current)
- Valid driver’s license; reliable transportation and auto insurance
- Minimum of two years’ experience as an Occupational Therapist in an acute care or rehabilitation setting
- Education: Bachelor of Science in Occupational Therapy from an accredited program
- Ability to work in a home care environment and to travel locally as needed
- Flexible schedule capability and willingness to travel locally
