Guide patients and their families through understanding therapeutic goals, discharge plans, and essential home exercises.

Maintain detailed documentation of evaluations, treatments, and patient progress in compliance with facility regulations.

Participate in collaborative care team rounds and interdisciplinary meetings for synchronized care delivery.

Doctorate in Physical Therapy (DPT) or equivalent credential from an accredited program.

State licensure as a Physical Therapist.

CPR certification preferred.

Specialized certifications (neurological, geriatric) are a plus but not mandatory.

Previous clinical experience is strongly preferred.
