Home Health Nurse

Position Type: PRN or PartTime
Schedule: Flexible

Position Summary

We are seeking compassionate and dependable Home Health Nurses (RN) to join our growing team. This role is ideal for clinicians who enjoy working independently, providing oneonone patient care, and supporting patients in the comfort of their homes. Prior home health experience is preferred but not required we welcome nurses who are eager to learn and grow.

Key Responsibilities

• Perform skilled nursing visits including assessments, medication management, wound care, and patient education
• Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
• Document all care accurately and timely in the EMR
• Communicate changes in condition to the clinical team and physician
• Educate patients and caregivers on disease management, safety, and wellness
• Collaborate with interdisciplinary team members to support patient goals and outcomes

Qualifications

• Active RN or LPN license in the state of Indiana
• Home health experience preferred, but not required
• Strong assessment and criticalthinking skills
• Reliable transportation and valid drivers license
• Ability to work independently and manage time effectively
• CPR certification
• TB Test Required
• Compassionate, patientcentered approach to care
