Home Health Aide Patient Escort

Brooklyn Center is actively seeking Home Health Aides to serve as Patient Escorts for our Skilled Nursing Facility in Brooklyn, NY.

Duties:

Escort residents to and from appointments
Observing and monitoring residents.
Reporting any health issues to the supervising nurse.
Maintain confidentiality of all resident & facility data.

Requirements:

Valid New York State HHA Certification.
Occasional heavy lifting may be required.
Ability to follow the Plan of Care precisely!
Must be able to work as a team member
Valid photo ID
