Maximus is seeking a compassionate Clinical Assessor, Registered Nurse to conduct in-person assessments in members' homes, hospitals, and community settings, evaluating medical, functional, behavioral health, and social needs. You will develop person-centered service plans, coordinate with healthcare providers, educate members and caregivers on resources, advocate for vulnerable populations, and maintain accurate documentation to meet Medicaid and LTSS requirements; travel Monday–Friday within #J-18808-Ljbffr
