Employment Type: Full time Shift: Description: - Home Care Registered Nurse (RN) – MercyOne/Trinity Health Home Care, Mason City Monday - Friday 8am-4:30 pm; Weekend rotation based on how many nurses are employed. MercyOne/Trinity Health Home Care in Mason City is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated home care team. In this rewarding role, you’ll deliver high-quality, personalized care to patients in the comfort of their homes, helping them maintain independence and improve their quality of life. What You’ll Do Perform comprehensive patient assessments and identify care needs. Develop and implement individualized care plans in collaboration with interdisciplinary teams. Administer medications and treatments, monitor patient responses, and adjust care as needed. Educate patients and families on disease management, medications, and wellness strategies. Monitor vital signs and manage chronic conditions with timely interventions. Accurately document all care activities and maintain compliance with healthcare regulations. Coordinate care with physicians, therapists, and other healthcare professionals. Provide emotional support and guidance to patients and their families. Participate in ongoing education and quality improvement initiatives. What You’ll Need Active RN license in the state of Iowa. BSN preferred; ADN accepted. Previous experience in home health or community nursing is a plus. Strong clinical assessment and critical thinking skills. Ability to work independently and manage time effectively.
