This role involves planning, organizing, and coordinating home care services, with a strong emphasis on community health and patient education.**Key Responsibilities*** Develop and initiate individualized care plans in collaboration with patients and caregivers* Deliver physician-ordered nursing services and implement preventive and rehabilitative procedures* Educate patients and caregivers to promote health and independence* Identify and implement discharge planning needs* Coordinate necessary equipment and services* Supervise LPNs, LVNs, and HHAs**Qualifications*** Active California RN license* Minimum 1 year of direct patient care experience (home health preferred)* Current CPR certification* Valid driver’s license and insured, reliable vehicle**Seaport Scripps Home Health**is part of the Pennant Group, a nationwide network with over 300 locations.
