Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.MINIMUM REQUIRED QUALIFICATIONS – CO/compact RN licenseBLS2 years case management experience in an acute hospital settingAcute hospital experience in specialty during the past year with no more than a 90-day breakPREFERRED QUALIFICATIONS – ED experiencePalliative Care Knowledge and GOC ConversationLENGTH OF ASSIGNMENT – 13 weeksSHIFT / HOURS PER WEEK – 8A - 4:30P with some weekendsSYSTEMS – Meditech/Midas/NavihealthSTART DATE – 8/3/2026Facility LocationAn amphitheater close to the Civic Center in Englewood is the focal point for visitors of this Rocky Mountain town where many summer concerts take place.
