Mountain Grove, United States | Posted on 09/09/2024 Position Name: Registered Nurse Hospice Case Manager Location: Mountain Grove, Missouri Specialty: Home Health Hospice Start Date: ASAP EmploymentType: Full-time, Travel,Contract, Temporary ContractLength: 8+ weeks contract -AcceptingLocal and Travel Candidates Pay Rate: Range $2080 to $2120 Weekly Gross pay (Non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers) MileageReimbursement: $0.49/mile for distancecovered between patient home appointments Unit: Home Health Hospice Shift: 8 Hours; Days, 8a – 5p Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri Position Requirements: Registered Nurse: 1 year - Required Recent/Current HomeHealth Nursing experience: 1 year within the last 3 years – Required Recent/CurrentHospice Nursing experience: 1 year with the last 3 years – Required Home Care Home Base(HCHB) charting experience – Required OASIS experience - Highly preferred Must have validDriver’s License and Personal Auto Insurance Must be CovidVaccinated or provide medical or religious exemption Education, Licenseand Certification Requirements: ASN or an equivalent– Required BSN or an equivalent– Highly preferred RN License for State applying or Compact if applicable- Required BLS Certificationfrom AHA or American Red Cross - Required Job Description: Provide andcoordinate care for assigned hospice patients in comfort of their homes. Manage assigned case load Develop and implementnursing care plans including Start of Care and admissions as assigned. Support patientrecovery and ability to stay at home - enabling them to spend quality timewith their loved ones and doing the activities they enjoy. Empower patients andfamilies to reach healthcare goals by educating them on disease management. Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures. Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure. Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented. Notify physicians ofsignificant changes in status. Document and completecharting for all patient visits within 24 hours. Provide bereavementsupport to patient’s families as needed.
