Key Responsibilities\n

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring they align with client goals and family values.

• Monitor and assess client outcomes while collaborating closely with a multidisciplinary team to ensure effective service delivery.

• Facilitate supportive services for families, helping them navigate challenges related to child development in a compassionate manner.

• Utilize evidence-based practices and Natural Learning Environment Principles to provide interventions tailored to the unique needs of each family.

• Provide coaching and support to families, empowering them with skills and strategies to enhance their child's development.

• Ensure compliance with all relevant federal and state regulations to maintain the highest standards of care.

• Foster meaningful relationships with children and families to create a nurturing and effective learning environment.

Patient Population / Client Focus\n

The LCSW will primarily work with children and families involved in hospice care, addressing complex emotional and social issues that arise during challenging times. Our focus is on supporting families facing end-of-life situations, ensuring they have access to psychological and practical support. Qualifications\n
