Position Overview

This position involves providing individualized care for residents in a skilled nursing facility (SNF). The selected candidate will work collaboratively with nursing staff, residents, families, and other departments to ensure optimal care.

Key Responsibilities

• Receive and provide comprehensive reports on residents' conditions, ensuring clarity on the care to be provided.
• Conduct rounds on each resident, assessing new residents and those who exhibit illness or have experienced falls or injuries.
• Document all assessments in accordance with facility policies.
• Notify nursing staff of any changes in residents' conditions or needs.
• Assist with direct patient care, ensuring prompt responses to call lights.
• Foster positive working relationships with other departments, team members, residents, families, and physicians.
• Carry out additional duties as assigned.

Schedule Requirements

The position requires availability for shifts on days, nights, weekends, and holidays. Candidates may also be required to float to other units as necessary.

Qualifications

• Must possess a valid California CNA license.
• Strong communication skills and a collaborative team player attitude are essential.
