Home Health Aide
Job Summary:

Responsibilities as a Caregiver:
Provide one-on-one support to clients in their homes, assisting with daily living activities such as bathing, dressing, and grooming
Assist with managing medications, incontinence care, and other personal care needs (as needed)
Monitor and report changes in client condition to the Care Manager and/or supervisor
Collaborate with the care team to develop and implement individualized care plans
Maintain accurate and timely records of client care and services provided
Requirements:
High school diploma or equivalent required
Must pass a background check and obtain TB test
Ability to lift up to 50 pounds and stand for extended periods
Strong communication and interpersonal skills
Compassionate and patient demeanor
Ability to work in independently and prioritize tasks accordingly
