About the Role:

As an Advanced Practice Registered Nurse (APN) Skilled Nursing, you will provide specialized medical care to our residents with complex needs. You will work closely with our interdisciplinary team to develop and implement individualized care plans, conduct assessments, and provide medication management and education. Your expertise will enable you to identify and address potential health concerns, ensure timely interventions, and promote optimal health outcomes.

Responsibilities:

• Conduct assessments and develop individualized care plans for residents with complex medical needs

• Provide medication management, education, and oversight to ensure accurate and safe administration

• Collaborate with the interdisciplinary team to identify and address potential health concerns

• Conduct wound care and other specialized treatments as needed

• Monitor and report resident progress and concerns to the healthcare team

• Participate in quality improvement initiatives to promote optimal health outcomes

Requirements:

• Current APN licensure in the state of Texas

• Master's degree in Nursing or related field

• Completion of a Nurse Practitioner program and certified as an APN

• Strong clinical knowledge and assessment skills

• Excellent communication and collaboration skills

• Ability to work in a fast-paced environment and adapt to changing priorities
