Job DescriptionPOSITION SUMMARY: To ensure the safety, stability, behavioral health treatment and movement towards permanency for children with special emotional, behavioral or developmental needs. To train foster and biological families in how to work effectively with the children placed.
DUTIES AND RESPONSIBILITIES:
- Maintain an assigned caseload of children who are placed in Community Residential Rehabilitative (CRR) - host home, TFC and/or Specialized Behavioral Health (SBH) levels of foster care
- Conduct ongoing social work assessments (e.g. Family Assessment Stress and Needs Tool (FAST), Ages and Stages, Child Adolescent Needs and Strengths) of children, their foster and biological parents through home visits, school visits and other community contacts
- Provide direct counseling and support services to children and their foster and biological families
- Work cooperatively with referring agencies (e.g. Department of Human Services (DHS), Community Behavioral Health (CBH), Community Umbrella Agencies (CUA)) and other team members to fulfill the permanency plans for each child
- Perform case coordination by linking youth and families with referring agencies and community services
- Facilitate team meetings and maintain communication with families and other members of the treatment team via phone, emails and face-to-face contact
- Provide ongoing documentation of work with clients including writing individual service notes and plans, individual crisis plans, quarterly reviews and treatment plans for youth
- Monitor foster parents to ensure compliance with the regulations, standards and policies and procedures of the program depending upon the level of care required by the child including providing medication logs, conducting visual inspections of foster homes and monitor biological family visits (only CRR) as indicated
- Conduct support groups (e.g. life skills session) for youth placed in foster care
- Complete face to face contact notes, quarterly resource parent support plan and foster/resource parent quarterly home safety checks
- Participate in the TFC on call rotation, as assigned
- Perform cardiopulmonary resuscitation (CPR) and crisis intervention using agency-trained protocols
- Perform other duties as assigned
EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS :
- Master's degree in Social Work (MSW) or master's degree in psychology, education or related human service field OR Bachelor of Arts in human service field with 2 years' experience counseling children and families
- Demonstrated ability to work effectively as part of a team
- Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
- Demonstrated strong attention to detail
- Demonstrated strong time management and organizational skills
- Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
- Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
- Must be able to perform CPR and crisis intervention using agency-trained protocols
- Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
- Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint
- Knowledge of Electronic Health Record (EHR)
- Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
