Work Environment This position is performed onsite, primarily within the community settings of our patients' homes. Key Responsibilities Deliver evidence-based orthopedic therapy tailored to each patient in their home environment across assigned areas in New Jersey. Collaborate closely with in-house orthopedic MDs, occupational therapists (OTs), doctors of chiropractic (DCs), and outpatient physical therapists to formulate and adjust integrated care plans. Assess patients' safety, mobility, and functional capabilities in the home, adjusting treatment interventions as necessary to optimize recovery. Educate patients and their families on appropriate exercises, body mechanics, and adaptive strategies to promote independence and enhance recovery. Utilize secure and user-friendly electronic medical record (EMR) tools to document therapy sessions, ensuring compliance with regulatory standards. Participate in interdisciplinary case reviews and contribute to ongoing clinical education and professional development. Patient Population / Client Focus The Homecare Physical Therapist will primarily work with patients who are recovering from orthopedic surgeries or managing chronic musculoskeletal conditions. This role is vital in providing personalized care and rehabilitation strategies that consider the unique challenges faced by each patient in their home setting, ultimately aiming for optimal functional restoration and improved quality of life. Qualifications Licensed Physical Therapist in the State of New Jersey or eligible for licensure. A minimum of 2 years of clinical experience in physical therapy, preferably in homecare or outpatient orthopedic settings. Exemplary interpersonal, communication, and organizational skills. Ability to work independently, manage a flexible schedule, and maintain a patient-centric focus. Passionate about delivering individualized, patient-centered care that promotes recovery and independence. Preferred Experience Experience working in a homecare setting or a multidisciplinary clinic is preferred, though confident new graduates with a strong educational background are welcome. Familiarity with EMR systems and the ability to adapt quickly to new technologies will be advantageous.
