RN Case Manager (CCM required)

RN Case Manager – Temp to Perm Location: Glasgow, KY. Pay: $55/hour. Schedule: Mon‑Fri 8am‑5pm with on‑call required one weekend per month. Shift Requirements Monday through Friday 8am‑5pm. On‑call required one weekend per month. Mileage reimbursement: 0.43 cent per mile. Coverage Counties Nicholas, Montgomery, and Menifee Counties. Preferred Skills PICC Wound care Negative pressure wound therapy Home health experience HCHB Primary Responsibilities Perform comprehensive assessment of high‑risk members. Collaborate with primary care providers to implement individualized, comprehensive care plans with specific interventions designed to engage the member. Address, identify, and continuously reassess cost‑efficient, appropriate levels of care. Provide non‑medical support (housing, transportation) to ensure compliance with treatment plans. Coordinate care transitions, including communication with physicians, social workers, discharge planners, and claims professionals. Engage specialty resources as needed to achieve optimal resolution. Keep detailed records of clinical, functional, and fiscal outcomes during the management process. Identify opportunities for health promotion and illness prevention. Prevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomes. Act as patient advocate by protecting privacy and confidentiality issues. Qualifications Associate's or Bachelor's Degree in Nursing or related field. Current, unrestricted state Registered Nurse license (KY). Compact license accepted. Analytical ability to assess complex medical information and make rational decisions. Case Management certification. Home Health experience with HCHB EMR charting. Participate in on‑call rotation. Background check, drug screen, and updated immunizations required.
