Full-Time Physical Therapist (PT) – Split Opportunity
Locations: Aventura at Carriage Inn & Aventura at Shiloh Springs
Location: Dayton &Trotwood, OH
Setting: Skilled Nursing / Long-Term Care (SNF/LTC)

Aventura at Carriage Inn and Aventura at Shiloh Springs are seeking a Full-Time Physical Therapist (PT) to join our energetic and supportive rehab team. This split opportunity offers the chance to work across two nearby facilities while making a meaningful impact on the lives of residents every day.

Responsibilities:

• Evaluate and treat residents in a SNF/LTC setting
• Develop and implement individualized therapy treatment plans
• Help residents improve mobility, strength, balance, and overall function
• Collaborate with nursing, therapy, and interdisciplinary teams
• Complete documentation using Net Health and PointClickCare (PCC)
• Maintain compliance with facility and regulatory standards

Qualifications:

• Licensed Physical Therapist (PT) in the state of Ohio
• SNF/LTC experience preferred, but not required
• New graduates are welcome to apply
• Strong communication and clinical skills
• Positive attitude and team-oriented mindset
