Lead DSP Medical Coordinator

Job Summary Mount Olivet Rolling Acres (MORA) is a nonprofit supporting adults, seniors, and children with developmental disabilities and medical conditions in the Twin Cities metro area. The Lead Direct Support Professional Medical Coordinator assists clients with daily living activities, personal care, medical needs, and program implementation in a residential setting. Responsibilities include daily client supervision, household maintenance, health care coordination, and maintenance of medical records. Schedule Week 1: Mon 3pm-11pm, Tues 3pm-11pm, Wed 7 hours flex, Sat 9am-11pm. Week 2: Sun 8am-9pm, Mon 3pm-11pm, Tues 3pm-11pm, Wed 7 hours flex. Hourly Pay Rate $20.25 per hour. Weekend differential +$2.00 per hour for hours between Fri 10pm and Sun 10pm. Overnight differential +$1.00 per hour for hours between 10pm and 6am.

• 12.13 per hour for sleep hours only when assigned.
• 1,500 retention bonus for full‑time staff & $750 for part‑time staff. Paid out over 1 year.

Essential Job Functions and Key Duties and Responsibilities Assist clients with daily cares, medical needs, ADLs, and mental health supports. Help clients maintain a daily routine, including hygiene, domestic tasks, leisure, grooming, socializing, budgeting, safety, and homework when applicable. Plan and implement recreation, leisure, and community activities. Drive to client appointments and community activities in company vehicle, covering all hours of operation. Support community relationships and facilitate friendships for clients. Implement programs in accordance with Coordinated Service and Support Plan (CSSP), CSSP‑A, and Individual Abuse Prevention Plan. Provide person‑centered services through practice, action, and language. Maintain knowledge of client history, skills, vulnerabilities, habilitation plans, and emergency procedures. Assist in developing and implementing goals as part of the support team. Administer prescribed medication, treatments, diets, and exams per CSSP directives. Maintain confidentiality per HIPAA and MORA policies. Prepare meals per planned menu and complete weekly food ordering. Assist with daily housekeeping and household maintenance. Complete house quality improvement assessments and address issues. Ensure adequate supervision and remain on shift until a replacement arrives. Work harmoniously with coworkers and exhibit respect. Communicate effectively verbally and in writing, using clients’ primary spoken language. Complete mandatory training as required by MORA or 245D regulations. Read and follow company policies and procedures. Remain awake at all times except during assigned sleeping shifts. Document program data and daily logs accurately and timely. Collaborate with the team to complete additional duties as needed. Additional Essential Job Responsibilities Schedule and attend client appointments; prepare referrals and documents for clinicians or external parties. Oversee medical appointment information exchange; document updates or concerns for the IDT, including the MORA nurse and house team. Oversee the medication administration record (MAR), ensuring current medication lists and information are up to date. Order medications, prepare new MAR each month, file old MAR, check medications weekly, and remove expired medications. Ensure safe use of adaptive medical equipment (DME) for clients. Attend medical specialist trainings and meetings. Flexible Hours Flexible hours are used based on client needs, including annual meetings, medical appointments, day program closures, oversight of administrative tasks, training, onboarding, performance evaluation, house audits, licensing, and other relevant duties at the home. Flex time is only worked on‑site unless approved by a supervisor for off‑site business purposes. Desirable Qualifications Interest or experience working with persons with intellectual and developmental disabilities in a healthcare oversight capacity. 3–5 years of experience with clients requiring medical coordination. Proven leadership skills. Strong team collaboration supporting client healthcare needs, appointment management, and coordination with the IDT and MORA team. Required Qualifications Pass Department of Human Services background check and maintain qualified status. Not excluded from working in government health programs by the Office of Inspector General. Physically able to meet company job demands. Valid driver’s license and vehicle insurance as required by state law. Successful completion of Medication Administration class with 80% or greater; regular proficiency in medication handling without error. Ability to use the computer in alignment with client needs and company procedures. Acceptance that job may include other reasonable duties as assigned.
