Clinical Pharmacist

Compensation Salary Range: $67.61 - $94.12 per hour Salary range above represents the lowest to highest possible rate. Factors that may determine placement within the range include specific skills, qualifications, relevant years of experience, and comparison to other employees already in this role. If you are viewing this posting on a job site, please visit our company page and search for the opportunity to view the pay range: Responsibilities Position Overview: Clinical pharmacist works daily and in coordination with Pharmacy Clinical Coordinator to support evidence based medication use throughout the health system. The Clinical Pharmacist will provide pharmaceutical care, including medication therapy consultation, monitoring, patient specific regimen optimization, adverse event investigation, and drug side effect mitigation. Additional program support duties include antimicrobial stewardship, drug education and training to Medical, Nursing and Pharmacy staff, and other projects and programs as designated by the pharmacy management team. Qualifications Required: PharmD or Bachelor of Pharmacy Degree Current CA Pharmacist license Pharmacy degree or equivalency exam Completion of practice‑relevant Board of Pharmacy Specialties (BPS) certification, e.g., Board Certified Pharmacotherapy Specialist (BCPS) Current Advanced Cardiac Life Support (ACLS) Certification Preferred: Pharmacy Residency Four (4) years acute care hospital pharmacy experience Benefits Community Memorial Healthcare offers competitive benefits supporting employees and families.
