Licensed Practical Nurse (LPN) - Skilled Nursing Facility

This is a 13-week contract position.

12-hour night shifts (6:45 PM - 7:15 AM), with weekends as scheduled. Every other weekend is required.

The Licensed Practical Nurse (LPN) will provide care to a mix of long-term and skilled residents in a Skilled Nursing Facility. Responsibilities include medication administration, wound care, foley/catheter care, PEG tube feedings, ostomy care, and tracheostomy care.

Administer medications and treatments as prescribed.

Conduct wound care and manage ostomy, PEG, and tracheostomy care.

Provide personal care and support to residents, assisting with daily living activities.

Collaborate with the nursing team to ensure comprehensive patient care.

Maintain accurate patient records and documentation using EMR systems.

Current LPN license in the state of practice.

BLS Certification (CPR must be completed prior to the first day).

Experience with IVs, PICC lines, mediports, and central lines preferred.

Familiarity with EMR systems, specifically Point Click Care.
