Job Summary
Physical Therapist (Outpatient) Oak Park, IL. The outpatient physical therapist evaluates assigned patients and develops, implements, and updates individualized treatment plans based on patient needs, goals, and physician direction. The role supports effective progression toward discharge, maintains accurate clinical documentation, and provides education to patients, families, students, and coworkers. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Days shift; 8 shift hours; minimum 40 hours per week; 2 evenings.
• Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ year of outpatient clinical experience; CPR certification; active/applicable license for the state where work will be performed.
• Preferred Qualifications: Prior successful completion of an assignment with the organizations agency; prior successful completion of an assignment with the organization.
• Responsibilities:
  • Complete comprehensive evaluations of assigned patients and apply clinical reasoning to develop treatment plans and goals focused on discharge within an established length of stay.
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats in compliance with state, federal, and intermediary guidelines and clinic policies and procedures.
  • Ensure care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care.
  • Participate in student programs as assigned, providing supervision, development of learning objectives, and student evaluations.
  • Create and maintain an individualized professional development plan; identify and initiate professional growth through continuing education, self-study, and professional activities and organizations.
  • Adhere to the discipline-specific State Practice Act and other regulatory agencies; maintain professional licensure in good standing and complete state-mandated continuing education requirements.
  • Perform in an ethical manner in accordance with discipline-specific professional codes of conduct.
  • Support organizational performance improvement by following action plans and offering feedback through staff meetings, focus groups, or direct supervisor input.
