Job Overview Department: 34124 Carolinas Medical Center - Pediatric: Hospital at Home Status: Part time – Hours per week: 24 Schedule: 8a–8p, 2 days per week, every 3rd to 4th weekend Pay Range: $38.20 – $57.30 Benefits Eligible: Yes Responsibilities Use the nursing process to assess patient needs, plan and implement individualized interventions in the home, and evaluate the effectiveness of the plan of care. Include the patient and family in planning and implementation of care, developing and maintaining a therapeutic relationship throughout the healthcare continuum. Anticipate and recognize significant changes in the patient’s condition, status, and need for care escalation in the in‑home setting. Provide in‑home assessment and take appropriate actions when changes occur. Alter the plan of care and mobilize resources as needed, collaborating with team members to coordinate patient‑centered care. Implement strategies to reduce patient risk and increase safety while in the home. Actively coordinate the discharge plan using an interdisciplinary approach. Apply evidence‑based practice to deliver patient care in the home. Consistently incorporate nursing and regulatory standards of care into practice. Provide ongoing in‑home evaluation of patient, team, and unit outcomes, taking appropriate action when changes are needed. Support shared governance activities and initiatives to improve processes and patient outcomes. Demonstrate effective communication and promote collaboration among team members virtually and in‑person. Assume a leadership role in the care team and delegate appropriately to unlicensed personnel. Promote a culture of safety by identifying threats to patient and care team safety and intervening to prevent harm. Report patient safety events and near misses in a timely manner. Identify potential safety issues and assist in the implementation of corrective actions. Apply ethical decision making and demonstrate respect for peers and other clinical disciplines. Participate as an effective member of the patient care team while working in the home to formulate an integrated, unbiased, individualized approach to care. Participate in quality/process improvement initiatives to improve department functions and the in‑home patient care experience. Engage in professional activities that contribute to personal development and development of others; seek opportunities to be taught, coached, and mentored. Attend required meetings/educational programs and complete annual competencies in a timely manner. Demonstrate knowledge and skills necessary to provide in‑home care appropriate to the age of the patients served. Understand principles of growth and development over the lifespan, assess patient data, and interpret information needed to meet age‑specific needs. Qualifications Education: Completion of an accredited or approved program in Nursing. Certification/Registration/License: Registered Nurse license issued by the state of practice. Basic Life Support (BLS) certification (American Heart Association). Valid driver’s license with acceptable vehicle operator record; travel required within the designated geographic area. Pediatric Advanced Life Support (PALS) certification preferred. Minimum 2 years recent nursing experience in an acute bedside or emergency setting; pediatric experience within the last 3 years preferred. Demonstrated ability to provide holistic patient care in the home, recognize and respond to situational changes with confidence, timeliness, and flexibility. Proficiency in phlebotomy, IV insertion, and midline placement. Proficient technology skills, including use of EMR, email, and remote patient monitoring equipment. Excellent written, verbal, listening, and interpersonal communication skills. Ability to work independently while in the patient’s home and collaborate with a virtual care team. Comprehensive nursing assessment skills in an untraditional environment. Human‑relationship skills to develop rapport and maintain professional relationships with patients, team members, and physicians. Initiative and collaborative teamwork skills. Critical independent decision‑making ability. Strong organization, prioritization, and problem‑solving skills. Time‑management ability with multitasking amid frequent interruptions. Coordination, collaboration, delegation, and team‑building skills. Physical Requirements and Working Conditions Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently. May work in confined spaces. Must be able to lift up to 50lbs. from floor to waist, 20lbs. over the head, and carry up to 40lbs. a reasonable distance. Must be able to push/pull with 30lbs. of force. May be exposed to chemical and hazardous waste, blood, body fluids, and communicable disease; protective clothing and equipment required. Must have functional speech, vision, hearing, and fine‑hand manipulation skills for IV insertion and other procedures. Must operate all necessary equipment and travel to all areas within the service area under all weather conditions.
