This role collaborates closely with residents, families, and the interdisciplinary team to ensure quality care and successful outcomes.### ### **Key Responsibilities*** Assess residents’ **social, emotional, and family needs** and develop appropriate care plans* Provide emotional support, counseling, and guidance to residents and families* Assist residents in achieving the highest level of **independence and well-being*** Coordinate with community agencies and resources to meet resident needs* Support **discharge planning**, including referrals, equipment, and services* Schedule and coordinate outside services (e.g., podiatry, dental, psychiatric care)* Assist residents and families with **care decisions and transitions*** Maintain accurate documentation of social services interventions* Respond to concerns regarding resident belongings and personal needs* Collaborate with interdisciplinary team members to ensure quality care* Provide **staff training and in-services** as needed### ### **Qualifications*** Bachelor’s degree in **Social Work or Human Services** (required)* **MSW preferred*** Minimum **2 years of supervised social work experience in a healthcare setting*** Experience in **skilled nursing or long-term care preferred*** Strong communication, organization, and documentation skills* Ability to handle sensitive information with **confidentiality and professionalism**### ### **What Makes You a Great Fit** Compassionate and resident-focused Strong interpersonal and counseling skills Organized and detail-oriented Collaborative team playerWe are an equal opportunity employer.
