Job Description

Job Summary:

The Wound, Ostomy, and Continence Nurse provides specialized care focused on wound management, ostomy, and continence issues. Under the supervision of the Wound and Ostomy Manager, the nurse delivers safe, evidence-based, and patient-centered care to a designated patient population.

Responsibilities:

• Conduct comprehensive assessments of patients' wound, ostomy, and continence related conditions.
• Develop, implement, and update individualized care plans based on clinical findings and patient needs.
• Educate patients and families on wound care, ostomy management, and prevention of complications.
• Collaborate with healthcare team members and make referrals to other disciplines as needed.
• Monitor and document patient responses to treatments, adjusting plans to optimize outcomes.
• Participate in quality improvement activities, including incident and prevalence studies, to enhance care quality.

Qualifications:

• Valid NYS Registered Nurse license.
• Certified Wound Ostomy Nurse (CWON), Wound Care Certified (WCC), or equivalent certification from recognized boards.
• 3-5 years of healthcare experience, with a focus on wound and ostomy care.
• Proficiency in utilizing electronic health records and healthcare billing systems.
• Strong interpersonal skills and ability to work collaboratively with diverse teams.
