RN Case Manager: Home Health Care Coordinator

Kettering Home Care in Ohio is seeking a Registered Nurse Case Manager (RNCM) to lead one-on-one home-based care for patients, coordinating with physicians and an interdisciplinary team to ensure quality outcomes. The RNCM will work at the top of their license, educate patients and families, manage caseloads, and ensure safety and compliance. Schedule: Monday–Friday, 8:30am–5:00pm with on-call rotation evenings/weekends/holidays. #J-18808-Ljbffr Ashealthnet
