Licensed Practical Nurse (LPN)

Carobell is an intermediate care facility for members with intellectual and developmental disabilities. The LPN at Carobell must have a wholistic approach to health care as the job duties are comprehensive and varied. The LPN will be involved in the daily monitoring of the residential member’s health status. This is accomplished by making daily rounds on the residential units and personally observing and interacting with the members. The LPN also interacts with the direct care staff to follow up on reports of the member’s clinical issues or concerns noted by the direct care team. Further nursing support and care is provided 24-7 with on-call nursing availability during nights, weekends, and holidays on a rotational basis. The LPN will regularly evaluate the members medical issues to determine health needs and will strive to maintain optimal health for all members. This is accomplished by performing Annual & Quarterly Nursing Assessments, Quarterly Physical Reviews, monitoring vital signs, weights, lab results, and other assessment data, and by providing on-site observations of all members as needed. The LPN maintains up-to-date records and records all member health issues, interactions, assessments, and outcomes in the electronic clinical record. All issues identified are also immediately communicated to the (Nurse-In-Charge) Supervisor (LPN) or the Director of Nursing (RN). The LPN regularly confers with the Medical Providers as needed to review health issues and receive orders for medications and/or treatments. The LPN frequently interacts with the treatment team to assist in planning, coordinating, and delivering a plan of care to meet the needs of the Residential Members. The LPN applicant must utilize strong communication skills, good time management and organizational skills, problem solving skills, excellent teamwork, and collaboration skills, and must exhibit the ability to be independent and self-motivated. The LPN applicant must possess an extensive knowledge of basic health care procedures and best practice theory. The mission of Carobell is: “Empowering individuals to reach their fullest potential”.
