Physical Therapist - PRN

Join a compassionate home health team as a Physical Therapist, providing patient-centered care in patients' homes to promote recovery, mobility, and independence. Key Responsibilities Include Evaluating patients' functional status, mobility, and rehabilitation potential Developing and implementing individualized care plans in collaboration with the care team Providing hands‑on therapy to improve strength, balance, and independence Educating patients and caregivers on treatment exercises and safety strategies Monitoring progress, adjusting treatment plans, and supporting safe care transitions Collaborating with nurses, physicians, and other clinicians to ensure coordinated care Documenting patient progress accurately within electronic medical records Supervising Physical Therapist Assistants and supporting caregivers Qualifications Include Graduate of an accredited Physical Therapy program (DPT or Master’s) Valid PT license in the state of practice1-2 years of experience, preferably in home health Strong clinical assessment, critical thinking, and communication skills Current CPR certification Reliable transportation and valid driver’s license Ideal candidates are autonomous, passionate about patient outcomes, and excel in building trusting relationships through compassionate care. #J-18808-Ljbffr Health, Inc
