Experience with working with oncology patients and families are preferred. Knowledge of the breadth and accessibility of community resources. Thorough knowledge of health care systems and terminology. Working knowledge of HIPPA regulations. Self directed and able to organize and manage multiple tasks/projects simultaneously. Ability to promote programs and services to community. Extensive understanding of oncology care. Proficient in learning medical record systems (electronic and non-electronic). Written and verbal communication skills. Summary The Patient Navigator will serve as a Patient Liaison to help patients and family members “navigate” PMC’s continuum of care after a patient receives a cancer diagnosis. The Patient Navigator will work with the Diagnostic Imaging Department, Pathology Department, Medical Oncology Physicians, Radiation Oncology Physicians, Surgeons, Social Work, Nutrition and Case Management to assure timely access and appropriate intervention during the cancer patient’s care.
