This position supports the population seeking treatment for primary care, substance use disorder (SUD), general mental health, serious mentally ill and dual diagnosis. Engages with patients and works as part of the care team to close care gaps. Provides services through the Mobile Health Unit in community-based settings including shelters, schools, community centers, outreach locations, employer sites and other designated locations throughout Maricopa County. Supports population health initiatives, preventive screenings, vaccinations, chronic disease monitoring, outreach, care coordination, and transitions of care. Collaborates with providers, behavioral health staff, community health workers, and community partners to improve healthcare access and outcomes for underserved populationsDuties Include:Provides nursing services.
