Flexible Monday-Friday day schedule with on-call rotation. Job Description SummitWest Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community. This is an in-person home health position offering flexibility in how you structure your day, along with manageable caseloads and meaningful one-on-one care. Responsibilities Perform comprehensive patient assessments. Develop and update individualized plans of care. Provide skilled nursing care and appropriate interventions. Monitor patient progress and report changes to the care team. Educate patients and families on medications, treatments, and care plans. Coordinate care with LPNs, therapists, caregivers, and providers. Complete timely and accurate documentation in EMR. Participate in on-call rotation. Required Experience / Certifications / Licensure Active Registered Nurse (RN) License in the State of Colorado (or active NLC compact license). Valid driver's license, reliable transportation, and auto insurance. Strong assessment and critical thinking skills. Ability to work independently and manage a structured caseload. Clear, professional communication with patients and team members. Comfort with documentation and EMR systems (HCHB preferred). Home health experience preferred, but not required.
