Licensed Practical Nurse (LPN)Mystic Healthcare & Rehabilitation Center is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our nursing team. In this role, you will deliver direct patient care, administer medications, and collaborate with RNs and interdisciplinary staff to ensure the highest quality of care for our residents in a long-term care and rehabilitation setting.The pay rate is $38.00–$47.00 per hour, depending on experience, plus shift differentials.Shift differentials:3–11 PM: +$1.00/hr11 PM–7 AM: +$1.00/hrWeekends (All Shifts): +$1.50/hrKey responsibilities:Administer medications and treatments as prescribed and in accordance with facility policiesMonitor and assess residents for changes in condition; report findings to the RN or attending physicianPerform wound care, catheter care, and other skilled nursing proceduresTake and document vital signs, intake and output, and other clinical dataAssist with admissions, discharges, and care plan developmentCommunicate effectively with residents, families, physicians, and interdisciplinary team membersSupervise and delegate tasks to CNAs and other support staffRespond to emergencies and provide first-line intervention as appropriateMaintain accurate and timely documentation in accordance with state and federal regulationsFollow all infection control, safety, and HIPAA compliance standardsQualifications and requirements:Active Connecticut LPN license in good standing (required)High school diploma or GED required; completion of an accredited LPN program requiredLong-term care or skilled nursing facility experience preferredCPR/BLS certification (or willingness to obtain upon hire)Strong clinical assessment and critical thinking skillsAbility to work independently and as part of a collaborative teamExcellent communication and organizational skillsAbility to stand, walk, and lift up to 50 lbs for extended periodsWhat we offer:Competitive base pay: $38.00–$47.00/hr based on experienceGenerous shift differential payHealth, dental, and vision insurance optionsPaid time off and holiday payContinuing education and career advancement opportunitiesSupportive, team-centered work environment
