RN-Wound Care

Duties:

Under the direction and supervision of the Wound and Ostomy Manager, the wound, ostomy and continence nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients. Responsibilities include:

• Conducts health assessments focused on wound, ostomy, and continence related care.
• Performs focused physical assessment, develops plan of care based on the diagnosis and individual client needs. Recommends treatments as appropriate.
• Assist with the development, revisions, and evaluations of standards of care.
• Assess and document patient response to wound care, ostomy, continence, fistula and complex wounds treatment. Evaluate the quality and delivery of care provided by the clinical staff.
• Works collaboratively with the health care team and makes referrals to other disciplines as indicated.
• Teach, counsels, and assists individuals and families to assume responsibility for prevention of complications as they relate to patients with ostomies, wounds, or continence problems.
• Routinely providing comprehensive assessments of patients at risk for developing pressure injuries. Initiating preventative measures.
• Participate in Unit/Department Quality Improvement and Hospital Wide Q.I activities, i.e. Incidence, Prevalence Studies and Nursing Quality.
• Provide culturally competent patient, family and staff education consistent with cultural, ethnic, religious and other belief systems.
• Maintain the security and confidentiality of data and information in accordance with patient rights policies.
• Performing other tasks as assigned.

Skills:

Required Skills & Experience:

• Three to five (3-5) years' experience in healthcare.
• Ability to fully utilize BHMC's electronic record systems, equipment and other healthcare and billing systems relevant to this position.
• Good interpersonal skills.

Required Certifications & Licensure:

• Active NYS Registered Nurse license.
• Certified Wound Ostomy Nurse (CWON) issued by the WOCNCB; or Certified Wound Care Nurse (CWCN) issued by the WOCNCB; or Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM); or Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO).
