Job Title: Licensed Physical Therapist
Location: Hagerstown, MD
Pay: $2,446 per week
Shifts: M-F, 8am-5pm
Seven Healthcare are seeking a dedicated Licensed Physical Therapist for a contract assignment in Hagerstown, MD. This role specializes in home health, conducting assessments, treatments, and overseeing PTAs across all of Washington County. The applicant should enjoy working independently within patients' homes and hold responsibility for managing comprehensive therapy plans.

Key Responsibilities of the Licensed Physical Therapist
- Perform physical therapy evaluation and treatment based on physician orders/referrals
- Complete comprehensive evaluations using appropriate tools and procedures
- Review patient records for diagnosis, medical history, and contraindications
- Establish measurable goals and develop treatment plans for independence and function
- Regularly re-assess treatment effectiveness and patient progress
- Communicate patient needs and progress with the treatment team and family
- Consult with medical providers about patient evaluation and treatment
- Recommend and assist with securing adaptive equipment for patients
- Maintain and submit required documentation
- Adhere to Company policies, HIPAA, and professional practice standards
- Supervise Physical Therapist Assistants and Aides in patient care
- Participate in staff development programs and obtain continuing education
Requirements of the Licensed Physical Therapist
- Valid Physical Therapist license in Maryland
- Experience with Home Care Home Base preferred
