IDD Case Manager

Job Summary The Case Manager is responsible for coordinating and overseeing services for individuals in accordance with New Jersey Division of Developmental Disabilities (DDD) guidelines. This role ensures services are delivered effectively, remain compliant, and support the health, safety, and quality of life of each individual. Case Managers serve as the primary point of contact for assigned individuals and manage all aspects of their caseload, including service coordination, staffing, compliance, and communication. Key Responsibilities Service Coordination & Oversight Implement and monitor Individualized Service Plans (ISPs) Ensure services align with DDD authorizations and individual needs Maintain consistent communication with individuals, families/guardians, and Support Coordinators Address service gaps, changes in needs, and concerns promptly Caseload Management Manage both active cases (ongoing services) and staffing/pending cases (new referrals) Serve as the primary point of contact for all assigned cases Ensure continuity of care and service delivery Referral & Intake Coordination Process new referrals from intake through service initiation Collect and maintain required documentation (ISP, PCPT, SDR, AENF, etc.) Coordinate onboarding and timely start of services Staffing & Scheduling Oversight Assign and coordinate Direct Support Professionals (DSPs) Manage call-outs, coverage, and re-staffing needs Maintain scheduling and tracking through agency systems Ensure staffing stability and service continuity Compliance & Documentation Maintain accurate, complete, and timely documentation Monitor staff service notes and compliance requirements Track staff credentials, trainings, and background checks Support audits, incident reporting, and quality assurance processes Communication & Quality Assurance Maintain strong communication with all stakeholders Promote person-centered practices and community inclusion Identify and elevate safety or compliance concerns Regulatory Requirements Follow all DDD guidelines , Medicaid waiver programs, and agency policies Maintain HIPAA compliance and protect confidential information Qualifications Experience in case management, healthcare, or human services preferred Knowledge of DDD services and IDD population is a plus Strong organizational, communication, and problem‑solving skills Ability to manage multiple cases and deadlines Proficiency in documentation and compliance tracking Benefits Health benefits offered Employer contributes $100 per month toward benefits Work Environment In-office Monday–Thursday Remote Fridays #J-18808-Ljbffr Beyond Home Health Care
