The role involves delivering skilled nursing services, medication administration, patient education, and interdisciplinary collaboration. The goal is to support patients' comfort and quality of life in their homes.Key Responsibilities:Provide skilled nursing care per physician orders and individualized care plansMonitor and document patient conditions, symptoms, and responses to treatmentAdminister prescribed medications and treatments safely and accuratelyAssist patients with disease management, symptom control, and comfort measuresEducate patients and family members on medications, treatments, and ongoing care needsCollaborate with physicians, registered nurses, therapists, social workers, and other interdisciplinary team membersObserve and report changes in patient status promptlyMaintain accurate and timely electronic medical documentationPromote patient safety while maintaining compliance with state and federal regulationsParticipate in care conferences and contribute to individualized patient care planningDeliver compassionate emotional support to patients and their families throughout the continuum of careQualifications:Current Licensed Practical Nurse (LPN) license in Idaho or eligible for Idaho licensureGraduate of an accredited Practical Nursing programMinimum of 1 year of Home Health and Hospice nursing experienceCurrent Basic Life Support (BLS) certificationValid driver's license with reliable transportation and current auto insuranceAbility to travel throughout assigned service areasStrong assessment, communication, and organizational skillsPreferred Skills:Experience with electronic medical recordsExcellent time management and independent decision-making skillsCompassionate bedside manner with strong interpersonal communicationAbility to work independently while collaborating effectively with an interdisciplinary teamKnowledge of home health regulations and hospice philosophy of careCommitment to delivering exceptional patient-centered care
