Pediatric RN: Integrated Case Manager - Care Coordination

Children's Wisconsin is looking for an Integrated Case Manager in Milwaukee to coordinate transitional care for children and families. This full-time role requires an Associate's Degree in Nursing and at least 2 years of case management experience, preferably in pediatrics. As an ICM, you'll work closely with healthcare providers to ensure continuity of care and support for patients.
