Hospice Certified Nursing Assistant (CNA)The Hospice Certified Nursing Assistant (CNA) provides compassionate, hands-on care that supports the dignity, comfort, and quality of life of patients during end-of-life care. Working under the supervision of a registered nurse and as part of the interdisciplinary hospice team, the CNA delivers personal care and assistance with daily living activities, ensures the safe use of medical equipment, accurately documents services provided, and offers emotional support to patients and families. This role requires professionalism, empathy, and a strong commitment to patient-centered care in accordance with agency policies and regulatory standards.This position is serving patients throughout the Aurora, Oswego, and Yorkville area.Essential Duties:The following duties are normal for this position. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Working in collaboration with the supervising RN, to fulfill the defined patient-specific Hospice CNA Plan of Care.Collaborating with the care team as an extension of nursing services to ensure all patient needs are fully addressed.Delivering hands-on personal care and supportive services to assist with safe activities of daily living, in accordance with the established Hospice CNA Plan of Care, and in compliance with the applicable laws, regulations, and agency policies.Documenting care provided according to agency policy.Using Durable Medical Equipment safely.Participating in activities that promote patient safety, quality, and regulatory compliance.Participating in professional development and lifelong learning activities.Performing other responsibilities as assignedCompensation & Benefits:The pay range for this position is up to $23 per hour, depending on experience and qualifications. The ability to manage personal emotions and maintain professionalism in these situations is essential.While this role requires a high degree of independence, it also involves close collaboration with a multidisciplinary team including physicians, social workers, and chaplains. Regular communication and teamwork are essential to providing high-quality care.This role requires active participation in staff activities that promote teamwork, unity, and morale. The individual will collaborate with colleagues to foster a supportive and cohesive work environment.The individual must comply with all personnel policies, hospice care procedures, patient rights, and federal and state regulations, including HIPAA privacy laws and the scope of care provided in a hospice setting.The individual will have access to patient medical records, ensuring confidentiality and HIPAA compliance while using this information to provide high-quality, personalized care in line with hospice care protocolsEmployees are expected to follow all safety protocols, including infection control measures, to ensure a safe environment for patients, families, and staff.
