Position Summary: The Integrative Nurse Practitioner (NP) provides comprehensive, patient-centered primary care services within TAN Healthcare's integrated care model. The NP evaluates, diagnoses, and treats acute and chronic medical conditions while emphasizing disease prevention, health promotion, and patient education. Working collaboratively with physicians, behavioral health professionals, care managers, nursing staff, and other interdisciplinary team members, the Integrative NP delivers whole-person care that addresses the physical, behavioral, and social needs of patients.

Essential Job Responsibilities

Patient Assessment and Diagnosis

  • Conduct comprehensive patient assessments, including obtaining medical histories, performing physical examinations, reviewing diagnostic findings, and evaluating patient concerns.
  • Order, interpret, and evaluate laboratory studies, diagnostic imaging, and other appropriate diagnostic tests.
  • Develop accurate differential diagnoses and individualized treatment plans utilizing current evidence-based clinical guidelines.
  • Assess patients across the lifespan while considering medical, behavioral, environmental, and social factors that may impact health outcomes.

Primary Care Services

  • Provide comprehensive primary care services for patients across the lifespan.
  • Diagnose and treat acute illnesses, chronic medical conditions, and minor injuries within the scope of licensure.
  • Perform routine wellness examinations, preventive screenings, immunizations, and health maintenance services.
  • Prescribe medications, therapies, and other treatment modalities consistent with Texas APRN regulations and organizational protocols.
  • Refer patients to specialists, hospitals, emergency departments, or other healthcare providers when medically appropriate.

Chronic Disease Management

  • Collaborate with physicians, behavioral health providers, nursing staff, pharmacists, care managers, referral coordinators, and other members of the healthcare team to provide coordinated, patient-centered care.
  • Identify behavioral health concerns through appropriate screening and facilitate timely referrals for integrated behavioral health services.
  • Address social determinants of health through coordination with internal resources and community partners.
  • Participate in interdisciplinary care planning and case conferences as appropriate.

Patient Education

  • Educate patients and families regarding diagnoses, medications, treatment options, preventive care, healthy lifestyle choices, and disease management.
  • Encourage patient engagement in treatment plans and promote shared decision-making.
  • Provide culturally sensitive education tailored to each patient's health literacy and individual needs.

Documentation

  • Maintain accurate, complete, and timely documentation within the electronic health record (EHR) in accordance with organizational policies, regulatory requirements, and professional standards.
  • Document patient encounters, treatment plans, prescriptions, referrals, consultations, and follow-up care.
  • Complete all required documentation to support quality initiatives, reimbursement, compliance, and continuity of care.
  • Participate in the rotating on-call schedule as assigned and appropriately document telephone encounters and after-hours patient communications.

Community Engagement

  • Support community outreach activities and health education initiatives that improve access to care and promote community wellness.
  • Represent TAN Healthcare professionally during community events when requested.

Compliance, Quality, and Professional Responsibilities

  • Maintain compliance with HRSA Health Center Program requirements, FTCA risk management principles, HIPAA regulations, state and federal laws, and organizational policies.
  • Participate in peer review, quality assurance, risk management, and performance improvement initiatives.
  • Support organizational quality goals related to clinical measures, patient satisfaction, continuity of care, and access to services.
  • Maintain current licensure, certification, DEA registration, credentialing, privileging, and continuing education requirements.
  • Attend required meetings, training, and organizational activities.
  • Perform other duties as assigned.

Minimum Education, Qualifications, and Experience

Required

  • Master's or Doctoral degree in Nursing from an accredited program.
  • Licensed as an Advanced Practice Registered Nurse (APRN) in the State of Texas.
  • Current national certification as a Family Nurse Practitioner.
  • Current DEA registration and prescriptive authority OR willing to apply for DEA registration
  • Current Basic Life Support (BLS) certification.
  • Demonstrated ability to assess, diagnose, treat, and manage acute and chronic medical conditions.
  • Strong clinical decision-making, communication, and interpersonal skills.
  • Ability to exercise sound judgment while maintaining patient confidentiality.

Preferred

  • Experience working in a Federally Qualified Health Center (FQHC), community health center, or other safety-net healthcare setting.
  • Experience providing telehealth services.
  • Experience serving underserved, uninsured, underinsured, vulnerable, or medically complex populations.
  • Experience working within an integrated primary care and behavioral health model.
  • Bilingual English/Spanish preferred.

Physical Requirements

  • Ability to sit, stand, walk, bend, and perform routine clinical duties.
  • Ability to communicate effectively with patients, families, staff, and community partners.
  • Ability to travel between clinic locations as needed.
