Certifications: BLS - American Heart Association Must-Have: - ASN - Graduation from an accredited School of Nursing is required. - Ability to read and write at a level to fulfill documentation requirements. - Competence in performing technical skills. - Demonstrated ability to manage urgent/emergent resident care situations. Description: The RN utilizes an interpersonal process that uniquely focuses on the total individual and his/her role in their family, culture, and society. The RN utilizes the nursing process, which includes assessment, diagnosis, planning, implementation, and evaluation. The RN fulfills a resident advocacy role by ensuring the resident and/or their representative has adequate information to make health decisions. The RN coordinates total care and works with the resident to achieve or maintain their highest practicable level of function. The RN is guided by substantial specialized knowledge of nursing based on physiological and psychological sciences. The RN ensures that the quality of resident care meets legal and professional standards and acts as a role model to other members of the healthcare team. This position requires the full understanding and active participation in fulfilling the mission, vision, and values of the organization. It is expected that the associate will demonstrate behavior consistent with the core values. The associate shall support the goals and direction of the quality improvement process. The RN completes holistic, age-specific admission history, physical assessments, and nursing plans of care on all assigned residents in an ongoing manner to monitor for deviations. The RN analyzes and incorporates clinical data to formulate an individualized plan of care. The RN identifies and documents residents at risk for nutritional and functional status. The RN identifies and reports signs or symptoms of neglect or abuse related to any vulnerable populations. The RN sets priorities for and reviews the daily plan of care in collaboration with the resident, family, and other healthcare disciplines. Delegation to team members is based on resident needs, continuity of care, and staff knowledge. The RN communicates with physicians to ensure coordination of medical and nursing plans of care. The RN documents nursing interventions, residents' responses to care, and resident outcomes. The RN evaluates data to revise the plan of care to meet the changing needs of the resident. The RN identifies and documents resident/family learning needs and provides education based on assessment in collaboration with the resident, family, and interdisciplinary team members. The RN identifies and documents discharge planning needs and coordinates or delegates discharge activities. The RN maintains a working knowledge of relevant regulations so that documentation supports and reflects compliance. The RN demonstrates the ability to supervise, coach, counsel, and evaluate the performance of assigned staff.
