Job Summary

We are seeking an experienced RN to join the Wentworth primary care team in a float capacity, supporting Medicare Annual Wellness Visits across all 11 primary care offices. This individual will travel to a different office each day, independently conducting Medicare Annual Wellness Visits and completing preventative screenings. The schedule rotates on a biweekly basis, with the RN receiving advance notice of their assigned office locations. Practice locations include:

• Dover, NH
• Barrington, NH
• Lee, NH
• Portsmouth, NH
• Rochester, NH
• South Berwick, ME

Schedule: 10-hour shifts, 4 days per week (40 hours), Tuesday through Friday, with a 7:00 AM5:00 PM schedule on Tuesdays.

Essential Functions

- Deliver direct patient care and support, ensuring high nursing standards.

- Conduct patient assessments, devise care plans, and assist in outpatient procedures with a focus on patient comfort and safety.

- Educate patients and families on treatment plans, medications, and post-procedure care.

- Collaborate closely with healthcare professionals to coordinate comprehensive patient care.

- Administer medications, perform IV therapy, and maintain accurate patient records in compliance with regulations.

- Uphold infection control protocols and maintain a safe environment for patients and staff.

Qualifications

Education

Associate's Degree Nursing required

Bachelor's Degree Nursing preferred

Licenses and Credentials

Registered Nurse [RN - State License] required

Basic Life Support [BLS Certification] preferred

Experience

Clinical nursing experience 0-1 year required

Ambulatory care or outpatient experience 1-2 years preferred
