Our innovative Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping people heal at home, with dignity and purpose.We are proud to have ranked in Boston Globe's Top Places to Work in Massachusetts and USA Today's Best Places to Work in the United States since 2023!Position Details:Main Office Location: Westwood, MAService Area: Westwood, Quincy, Dover, Weymouth, Holbrook, Randolph, Brockton areas.Job Type: Per DiemResponsibilities:Initiate appropriate preventive and rehabilitative nursing procedures.Administer medications and treatments as prescribed by physicians.Conduct initial assessments of patients and families to determine home care needs.Perform thorough physical assessments and gather comprehensive medical histories.Develop and revise individualized plans of care based on patient status and evolving needs.Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.Responsively address inquiries and concerns from patients, caregivers, and medical teams.Provide counseling and support to patients and caregivers before, during, and after treatment.Supervise home health aides as required.Identify discharge planning needs and proactively implement them as part of the care plan development.Qualifications:Must hold an active Nursing license in the State where the position is based.Preferably 2+ years of nursing experience, with a focus on geriatrics.Nursing degree from an accredited institution.Excellent bedside manners and exceptional communication skills.Working knowledge of healthcare laws and regulations.Maintain current BLS CPR certification.Possess a valid driver's license and own a reliable, insured vehicle.Why Join Pathways?Work in a supportive team environment and amazing corporate culture.Impact patients' lives directly by providing in-home care.Opportunities for continued growth and professional development.Benefits & Compensation:Per Visit Rate ($50 per visit | $100 per admission)Mileage Reimbursement401(k) MatchFlexible schedulePathways Healthcare is committed to providing exceptional care to our patients and fostering a positive work environment for our team members.
